The Innovative Community Participation Program from Connected Self is made to help NDIS participants develop the skills they need to take an active role in their community.
The programme employs highly qualified Community Engagement Practitioners who work side by side with participants to increase their possibilities for employment and community involvement.
By collaborating with participants in their neighbourhood, community engagement practitioners utilise a capacity-building, community-based strategy to maximise the chance of uptake and a smooth transition from the programme to informal supports and/or employment
